A Health Care Directive is used to ensure that your medical wishes are carried out when you are unable to personally convey them.
A Health Care Directive contains both a Living Will (for health care instructions) and a Medical Power of Attorney (for appointing a health care agent to act on your behalf).
Depending on where you reside, the names of these sections may change but the functions stay the same.
